Picture Chairman Paperwork

Picture Chairman should print and fill out the forms located on the left of the page. Bring the completed forms with you on the day of photography.

Your packet of information will contain forms that your members need to complete and hand to your photographer on the day of your photo session. New members’ information must be included. Members who have already been photographed by us in prior years do not need to fill in camera card data unless they need to make changes.
